Application to Change or Cancel an Order (Form 2)
They’re all in the Procedure section under The Provincial Court. All the rest of the forms on the page (Supreme and Appeals) check out okay.

The person who starts a proceeding in the Provincial Court is the ''applicant''. The person against whom the court proceeding is brought is the ''respondent''.
